INFORMATION FOR PATIENTS

First consult visit

Please be aware that you may HAVE YOUR PUPILS DILATED on your first visit and if so, will experience blurry vision for up to 4-6 hours after your appointment. You should be accompanied by a driver and bring sunglasses to wear for comfort as you may see blurry or be sensitive to light after. 


                                                                                    *Please allow 1-2 hours for your appointment*


The dilating drops take 20-30 minutes to work. We try our best to see patients in a timely manner but we do see emergency patients whose visits may take longer than anticipated. Your patience and understanding is appreciated. 


We ask that you bring: 

  1. A valid health card- if you do not have an updated, valid health card at the time of your visit, you will be charged privately for the visit and it will be your responsibility to claim subsequent reimbursement from OHIP.
  2. An up to date list of your medications and any eye drops you are taking
  3. A translator if patient does not speak English or French
  4. A friend, family member or Power of Attorney if patient is unable to make informed decisions for themselves


We require 48 business hours to cancel or reschedule an appointment. If less than 48 business hours is given or if an appointment is missed, a $90.00 fee that is not covered by OHIP must be paid BEFORE another appointment can be rescheduled. It may be weeks or months before another appointment is made due to the volume of patients we care for. 


You will remain under the care of the referring physician or optometrist until seen and assessed by Dr Daniel.
